Are you collecting a UK pension? If so you need to obtain an S1 form.

Are you working self employed?

If your paying into Spanish social security, then you're entitled to register with them, and they'll allocate you with a number that will enable you to obtain a SIP card from a medical centre. Your employer should be able to help you with this.

I'm surprised that you mention NIE and SS but not residency. If you're resident without residency then you're illegal, so shouldn't expect to be given health cover. If you have residency and are paying into SS then there shouldn't be a problem.

Some medical centres will issue temporary SIP cards, but they aren't obliged to do so.
