Before taking out health insurance, I recommend you have a look at the coverages each company provides. Specially the medical network they offer as it depends what company you go to (some offer a limited medical network).
Also, for any type of VISA or residency, it is mandatory for you to have a fully comprehensive insurance, without copayments (which is stated by the Spanish consulate to accept your application).
If you only have a passport, most companies will ask you to pay the full year upfront, but if you have a NIE and spanish bank account, you will also be able to pay monthly premiums, (Our clients have never had a problem with the Spanish consulate about paying monthly premiums).
