When taking out private health insurance, in case of VISA or residency, you need a fully comprehensive insurance, with no copayments (it is mandatory from the Spanish Consulate) to be able to get accepted. Have a look for insurance companies with a wide range medical network as some companies have limitations.
I recommend that you go through an insurance advisor that also provides support on policy matters, you may need help with authorizations, etc.. Also for people over the 40s years old, I suggest to use an insurer that provides Lifetime guaranteed cover, which means that once you are accepted, the company can not cancel your policy. This is only offered by AXA and DKV in Spain as far as I'm aware.
