The insurance policy must be valid for 1 year and must cover all the beneficiaries of the visa application for the risks insured by Spain's public health system (cover that is equal or superior to that provided by the Spanish public health service) with no co-pay (or deductibles) contracted with an insurance company authorized to operate in Spain. No travel insurances with medical assistance coverage will be accepted.
There is a plethora of insurance companies who offer cover therefore it may be best to shop around.

You may be interested to know about a scheme known as the Convenio Especial. Once you have been legally resident in Spain for at least 12 months, you can apply to register on the Convenio Especial.
The Convenio Especial is a pay-in scheme for public health care services delivered by the Spanish regional health authorities. For a monthly fee (€60 under 65, €157 over 65) you’ll be covered by the Spanish public healthcare system, including pre-existing conditions, although you will have to pay for prescriptions and some extras like prosthetics.

I can highly recommend ASSSA for private medical insurance that satisfies the requirements for a NLV.
