Please see the advice I posted a couple of days ago

Presuming you are under pension age and not registering for a permanent SIP card with a S1 form and that you just require a temporary SIP card to receive your vaccine.

You will need to register for a temporary SIP Card at your nearest salud. Take your passport and padron with you. Different saluds ask for different things so I would also take copy of your visa, private health insurance and your EHIC. They may ask for just one or all three.

Explain you need the temporary SIP for vaccination purposes.

Spain are giving boosters to the 70+ age group at present. The roll out of boosters for the 60 -69 group looks as though it’s been delayed by a few weeks as they are next week rolling out the vaccine to the 5-11 age group.
