Maybe you could just go along and ask if they speak English, sometimes they do but prefer us to bring a translator to make it easier for them, especially if it's a medical appointment..
You could possibly use google translate to write down that you are sorry but could not get a translator to come today. As it sounds like it may be just a signing on appointment and not an actual medical appointment it may be OK. If it is medical then it's more important to have a translator to avoid any misunderstandings but unless she is ill, it may be OK not to have translator on this occasion (??).
