It varies depending on your circumstances.
Without going in to details.
Presumably you have already obtained your residencia (or TIE) and not working. You will also require an up to date padron certificate.
If you are in receipt of a qualifying state benefit (ie pension etc) you need to apply for an S1 form via your pension provider. This transfers your Irish/UK (etc) medical care to Spanish healthcare.
If you donât qualify via state benefit you can apply to pay for state healthcare, rather than private healthcare.
If you can advise which of the above applies to you, we can then point in the right direction.

Are you entitled to free healthcare in Spain? Are you receiving a State Retirement Pension from the UK or Ireland, for example? If the answer is yes to the second question you need to contact the NHS (NHS Overseas Healthcare Services) in the UK and request form S1. Once received you can register your application online with the Spanish Health Service. The following links will provide you with further information:

Thanks for your reply, I don't have residencia but I do have an NIE number, I am aged 67 and receive a contributory pension in Ireland and a small UK pension, I don't have a padron but shouldn't have any problem getting one, I don't have health insurance in Ireland.

At 67 you're entitled to a S1 from Ireland to help to apply for residency in Spain. You'll also need proof of funds, currently 1 X PREM, which is a lot less then non EU countries.
Once you're registered on the padron & have residency, then you can register your S1 for Spanish healthcare.

You haven't said if you are, or want or plan to become, resident because without that there can be no SIP card or Spanish health cover.
You make light of getting a padron but that too is predicated on residency.
An NIE of itself means nothing, anyone can get that resident or not.

So to answer your initial question, to obtain your Sip card you need first to become a resident.
You can only go on the padron if you intend to become a resident - you have to fill the criteria to be able to apply to go on the padron. Depending on how your ayuntamiento issue padrons, that can be an extremely stressful experience! As per Darroâs reply - once you enter the world of Spanish red tape you will find that nothing is as easy as it seems.
Only once youâve got your residencia can you then apply for a SIP card for healthcare.
