Travelling to the UK

Please can you tell me what I would need to do before flying to the UK (Wales) before Christmas?

Fill in the attached form
Self isolate when you arrive.
There is no legal requirement (as I write) to have a -ve PCR before you leave for the UK, but it seems irresponsible not to do so
