You first need to apply for a visa from the Spanish consulate while you are still in the UK. You will need to meet the income and healthcare requirements. You cannot apply for residency until you have gained a visa.

That very much depends on what passport you hold. EU = easier, UK = more difficult. It also depends on why you were refused. If it was over finances, has this changed? If it was over  lack of health care, have you sorted this out?

Did you have a solicitor or gestor to help you apply last time and did they give you any feedback? It could have been a minor issue or it could mean you will never be given residency but we need more information.
