NHS Overseas. The S1 is available to people in receipt of a State Pension and is mailed to your Spanish address. 

You take it to your local INSS Office (prior appointment required) and they issue the correct paperwork to take to your Centro Salud. If the NHSS staff speak English it's an unlikely bonus. Some people use a Gestor for that, the charge is typically in the region of €40-50. 

The Health Centre issues your SIP card on receipt of that paperwork and on the reverse you will find the name of the Doctor allocated to you.

Once you visit the Doctor, he she will determine what medication you need to have on repeat prescription. Bear in mind that repeats require occasional renewal. This can be done at the Centro Salud reception desk or on the phone.

Edit, just for info, a Ventolin inhaler costs €2.50, a Salbutamol in the region of €53.

You'll pay for your medication until you file a Spanish tax return which will then be used to determine your income level and consequently what level of discount you receive on medications.

We have applied for the convenio especial it's over a month since we sent the documents via our local town hall. We've been told it can take up to 3 months to hear anything - we only know the application was received at Alicante because the documents were scanned and sent over to the right department.

Not really what you want to hear during a pandemic, especially when you are trying to do the right thing.

I think, and this isn't a dig at the original poster, that some people are still trying to tip toe between the UK and Spain since Brexit and to avoid significant healthcare costs. For us as a mid 50's couple costs are Eur 3500 pa if we maintain both private and public healthcare (in order to close all gaps through pre-existing conditions) it isn't cheap.

Maybe for some 90 in 180 here would make more sense if residency doesn't really work.
