I was told that you did'nt?

If you've got a job with a contract and are paying into social security, then this qualifies you for health cover, so no need for PHI.

Yes, but a new autonómo can take advantage of the 'tarifa plana' scheme, where the starting rate of contributions is €60 a month (unless it's gone up recently). That increases progressively to the standard rate of ~€300 a month.

Is this over a year or so, so you can establish the business and register as a resident?

The starting rate of €60 is for the first year. Because of the mandatory need to register as a resident within three months, the authorities accept proof of three months paying the autonómo fee as satisfying the requirements for residency.
