Hospital / Health centre prescription

Hi, I'm not sure how the health centre can attend your need, but maybe you can give them a call, their phone nº is (+34)  965 757 666. They probably ask for your name and date of visit but couldn't say if somebody speaks English in there.

However, in spanish it could be something like this;

Mi nombre es (my name is) ....... y el día ...... me atendieron en su consulta por una tonsilitis prescribiendome un antibiotico. El caso es que mi empresa solicita un justificante de la prescripción que entregué en la farmacia por lo que agredecería me pudieran enviar una copia de la misma bien a mi email ...... o por fax al nº .....

Translation:  

My name is  ....... and the day ...... I was treated in your surgery for tonsillitis by prescribing an antibiotic. The case is that my company requests a proof of the prescription that I delivered in the pharmacy so I would be grateful I could send a copy of the same either to my email ...... or fax to the number .....

Hi its an email address we require for the Centro De Salud De Gata de Gorgos. I don't think my broken Spanish could stretch to the conversation required.   Really appreciate yours and anyone else's assistance.   Thank you

Hi, after contacting the health centre in Gata de Gorgos, they cannot provide any information over phone due to data protection law, but have been indicated that you can send a FAX to 0034 965 757 156 to get attended by pointing out your name, ehic card nº, date of visit and subject (maybe in two languages GB & E would be suggestable)
