None of the medical centres I know of require an appointment.

If by saying "swapping" you mean exchanging then you do not need a Medical Report, it is only needed if you are applying for a Renewal.

The nearest one in Gran Alacant is at the private Salvum Clinic in the Life Resort. If you are willing to travel, you can get one done much cheaper in Urb La Siesta near Torrevieja.

And yes, you do need a medical report if you are changing your UK licence to a Spanish one! 

The medical Report is only needed if you are Renewing, not if you are Exchanging.

Sorry John I can categorically state that at Traffico in both Alicante and Elche you WILL be asked for your medical report when exchanging your UK licence for Spanish. The last time I did this was in February this year.

And I can categorically state that when I attended the Trafico in Elche in February 2019 I DID NOT  require a Medical Report when Exchanging my U.K. Licence for a Spanish Licence. The difference is with Renewals, then you do need the Medical Report.

This is my experience of EXCHANGING my U.K. driving licence for a Spanish driving licence.

Booked the appointment on the DGT website in November 2018 and was given an appointment today ( 18/02/2019 ), selecting the Trafico Office in Elche. After some difficulty actually finding the Office ( need to invest in a Sat Nav LOL ) I attended the appointment, they have a ticket queuing system which you log into using your NIE Number and when called my application was dealt with by a friendly English speaking member of staff. The process was straightforward and at the end I was issued with a temporary licence until my new Spanish Licence is posted to my home address.

The documents required were

1. The Application Form, printed from the DGT website when making the application.

2. U.K. driving licence, original and copy

3, NIE certificate, original and copy

4. Residencia certificate, original and copy, 

5. Passport, original and copy

6. Photograph, recent, 1 measuring 32mm by 25mm

7. Payment of 28.30€

NO MEDICAL REPORT WAS REQUIRED FOR ME TO EXCHANGE FROM MY U.K. DRIVING LICENCE TO A SPANISH DRIVING LICENCE.

Definitely not for an exchange, only for a 'renewal'.
