As things stand at this instant it's possible that he might be able to get treatment under an EHIC but assuming a no deal Brexit on Oct 31st. UK EHIC's will become worthless not only in Spain but equally anywhere in the remainder of the EU.

Even if he were to secure treatment under an EHIC it's unlikely it would be a simple continuation of what he's had in UK, for something as dramatic as dialysis the Spanish would undoubtedly want to perform their own tests and examinations and determine treatment based on their own diagnosis.

Failing the above then the only option would be to pay privately, potentially very costly I would imagine.

Note: again assuming a no deal Brexit, they will only be able to stay in Spain (or anywhere in the EU) for a maximum of 90 days in any 180, so by the time he's gone through the process and started treatment in Spain he'd probably find himself out of time and having to leave and return to UK for at least another 90 days - then likely have to start all over again there.

With his needs then even paying privately I don't think the plan will be possible, or at least not until/unless UK exits the EU with a deal which at the very least permits them to stay for longer than 90 days. In reality for continuity of treatment they would probably have to take up residency.
