Scam perhaps?
Doubt it. DGT has a traffic office in Elche which does driving license exchanges and the OP is simply advertising a service which many appear to be a bit beyond them.

The OP is talking about exchanging the license, not simply renewing it. You still need the medical but he is offering to do the actual exchange from a foreign one to a Spanish one which is done at DGT either in person or by a nominee.

That´s correct. I support with the exchange of an EU or UK licence to a Spanish licence following obtaining resident status. I book the medical, register the client for the digital certificate service, obtain the dvla check code, submit the application, take the licence and forms in person as representative to Elche DGT then track and collect the spanish licence card from there around 4 weeks later which removes any postal issues to the clients address.
