You need a medical to renew a license, and the medical centre can process the renewal, so no need to bother going to Tráfico.

Without looking, I'd say Murcia is the nearest, then Cartagena.
Ours were originally done at Murcia, because they were more efficient than Alicante, and the Elche office hadn't opened at that point.
