Hi - Yes, I've just asked my vet about this as we are hoping to travel to Spain with our dog in January.
We were advised that she will need the usual annual vaccinations, but also a rabies vaccination that has to be at least 3 weeks before travelling. Finally, the vet needs to see her 10 days or less prior to travelling. They will check she has all these vaccinations, is in general good health, then they will issue an "Animal Health Certificate", which I believe replaces the old pet passport scheme. It takes the vet about an hour to put the certificate together and costs £150.
I think it is only good for 4 months throughout Europe, so might have to be renewed if you are staying longer, best to check this point, as I didn't take full note of that as we are only going for 2 months.
On returning from Spain, a vet in Spain will need to treat her for tapeworm 24-120 hours before the return journey.
