Here is a link you need to fill in your health declaration for covid 19 48 hours before you fly and get your QR code hope this helps

Hi DaveBev

Is this the same form that will be handed out before departing from theUK, and then handed in upon arrival at Spain? Also, the QR code that you recommend to install - is this to assist going through the temperature readers and, if so, what happens if you don’t have this code?

We haven't done this ourselves yet. I know the form can be printed off as well so probably the same info. The app would save time and can be completed before you travel.
