You need to be more specific. Application to register an S1 for health cover, application to sign up for payments into social security, application to apply for benefits, etc.

People who are starting up as self-employed - autónomo - go for the Tarifa Plana, which gives you a massive discount on social security payments, and you only pay €50 a month for the first twelve months. After that it goes up to, I think, €300 pm.

Once registered as autónomo, you can also register for health care.

I haven't got time to fully research the means of registration, although I've seen mention somewhere that you can do it online if you've got an electronic certificate, but the following seems to be a good starting point:
