We live outside of the town of Polop and having recently received our SIP cards. Are we obliged to use the Polop surgery or can we for example register at the La Nucia Health centre? La Nucia is as close to us as Polop and the health centre there seems to be far more comprehensive with a far wider range of facilities.

You can register at any Medical Centre if they are willing to take you on.

Thanks for your help - we originally went to La Nucia health centre and they sent us to Polop who have now given us a temporary SIP. Can we still change ?

I believe so as having a temporary SIP Card does not mean you are registered with a Doctor, go to you preferred Medical Centre and ask to be registered with a Doctor, possibly asking for one who speaks your language if you don't speak Spanish otherwise you may require a translator to attend appointments with you.
