I'm assuming that you are both UK passport holders. 

If you are under pensionable age then your NLV application will require a full years paid-in-full healthcare insurance.

I assume that you know the income requirements for a couple.....

When you reach pensionable age you can apply for the S1 healthcare cover which is paid for by the UK government. You may be required to pay a small percentage towards the prescription for meds.

The private health insurance is only required for the first 12 months of residency, after which you can apply for the "Convenio Especial" until you reach state pension age. Once you reach pension age and are eligible for your S1 then health cover is free.

The price you've been quoted seems expensive, try shopping around. We used ASSSA.

To be honest 250€ per month is not unreasonable. My partner and I pay 2700€ for private medical insurance annually in Spain even though I have access to the Public Health Service by way of S1 and my partner contributes to the Convenio Especial. Bear in mind private medical insurance does not cover pre existing conditions nor prescriptions. It will be a few years until you become entitled to State Pension and be able to apply for the S1 from the UK providing off course you have contributed to the UK National Insurance Scheme. Shop around and perhaps you might come up with less expensive cover. But remember private insurance has to cover all eventualities with no co payments. Once you have been officially resident in Spain for 12 months you can apply to join the Convenio Especial which is administered by the Spanish health authorities. This would give you full access to the Spanish health service similar to that avaiable to Spanish Nationals. The Convenio covers pre existing conditions but the cost of prescriptions must be met in full. The monthly subscription to the the Convenio Especial is 60€ per person, if you are under 65 and 157€ per person for 65 + The following link will provide you with details of the Convenio Especial.
