We are EU (Irish) living in Spain. Currently we have private healthcare, but would like to switch over to the Spanish local healthcare to cut down our costs. I believe the local public healthcare is cheaper, especially for myself as I am 65. The private healthcare cost for the over 65’s is very expensive, so looking for the cheaper options. We are both in excellent health, thank god. I will only get my Irish medical card end 2023, at 66 hopefully I can then switch it to the Spanish card. But for now any advice would be appreciated.

The attached link will provide you with information about the Convenio Especial which you can apply to join if you have been resident in Spain for at least 12 months. At the moment the cost of the Convenio Especial is 60€ per month if you are under 65 and 157€ per month if you are 65 and over. When you reach Irish state pension age you can then apply to the HSE in Ireland (HSE Overseas Section 016201816) for an S1. Once received you must register the S1 with the Spanish INSS and once accepted and processed, you will become entitled to free health treatment. Remember to have your partner added to the S1 as your dependant and they will be entitled to treatment also. When your partner reaches Irish state pension age they must apply to the HSE for their own S1and register with the Spanish INSS.  I have also attached a link to Citizens Advice Spain where you will find further information and a link to Age Spain where you will find step by step instructions on how to register form S1 online.

The critical point from the above reply is "if you have been resident in Spain for at least 12 months".

That means fully and legally tax resident and on the padron etc. not just living here.

You need to have been registerd as resident with the police for a minimum period of 12 months before you can apply to join the Convenio Especial.

Thanks John, Darro really appreciated. We are 10 months in the country with our NIE resident green cards. As we both work (remote) for Irish companies based in Ireland but live in Spain, and pay our taxes in Ireland. How can we register for Spanish tax. I thought Ireland &Spain had an agreement that if you are taxed in Ireland you don’t pay Spanish tax, avoiding been double taxed.

...nt that if you are taxed in Ireland you don’t pay Spanish tax, avoiding been double taxed.

If you live in Spain you pay your tax in Spain, it's not a matter of choice or preference.

Dual tax agreements exist to ensure you don't pay twice.
