Hello Buzby,
When taking out Healht Insurance for your residency, you need a fully comprehensive cover without co-payments, as this is a requirement from the Spanish consulate. Also, the insurance is a 1 year contract, since you have a NIE number all you will need is a Spanish bank account so you can pay monthly instead of the year upfront.
I suggest that you have a look at the companies Medical Network, as some are limited, in addition, if you are thinking of keeping the insurance for more than 1 year, I would recommend you have a look at insurance companies that have a LIFETIME coverage (this means that they can't kick you out in case of high medical expenses, age or serious illness (once accepted)), as far as I know only 2 companies have this in Spain: DKV and AXA.
Regarding the NLV, I suggest that you get a professional to help you, as the criteria changes frequently, the same goes for your insurance.
Feel free to contact me if you would like further advice, I will be happy to help. I work in an insurance agency through AXA.
I hope this helps!
