Being a UK citizen I know I require full private healthcare cover for the first 12 months. However my wife is European so does this mean she can straight into the Spanish NHS system.

If she's over State pension age she's entitled to an S1, but if not, then she will need health insurance

Free healthcare for those not in receipt of a state pension, registered as disabled, under 18 or pregnant is only available if you have paid into the Spanish social security system. Over retirement age and in receipt of a state pension will give you rights to the S1 to transfer your healthcare from you home country and your dependents (spouse etc) will receive their own S1 as a beneficiary of the S1 holder regardless of age.

Although saying you will need private health care for the first year, you will also need it for the first 5 years if not working but you can apply for the convenio especial after the first year of residency which is €60 per month if under 65 and €157 for those 65 or older and does not cover prescription costs but will cover all pre existing conditions. I know the UK gov site states you can get free healthcare when you become a permanent resident but that is not true unless you meet the requirements of receiving a pension or paying Spanish social security payments.
