Hello,

My name is Patricia.

I am ok with filling out pages 1 and 2 of the NIE Application form EX-15

But, Page 3 Has 2 large boxes Headed ANEXO 1 And ANEXO 2

ANEXO 1 Documents sobre los que se autoriza su comprobación o consulta - Which is the one I require

Which seems to be documents I am presenting for the application i.e. Passport

The first column is Documento - where for example Passporto would be entered

but there are 3 more columns headed Organo - Administration -Fecha de Presentacion

Organo means Organ so what do they mean by that regarding a passport

Perhaps Administracion is the Office that issued the passport - but I am not sure and

The last column Fecha de Presentacion, which means date of Presentation - Does that mean the date that the Passport was Issued?

So my questions are what goes under the 3 fields Organo - Administration -Fecha de Presentacion?
