When they issue you an S1, they do not remove you from the UK NHS system. You just get the form, take it to INSS near where you live inSpain and are given another form (037 or something like that). You then take that to the health centre where you will be registered with a SIP card. You are now on the Spanish NHS
Only then is the overseas department in UK notified by means of that form that you have registered in Spain and then they remove you from the NHS UK. There should be no gap in cover as you are not removed just by getting the S1.
We moved over in December and had applied for the S1 about a month before the move. The S1 forms were waiting for us at our address in Spain. We took out residency in February and applied to go on the Spanish NHS. We got the SIP card and were allocated a health centre and GP here in Spain.
The next month, we received a letter from the overseas health in Newcastle saying we could apply for an EHIC and that we would still be covered in UK as pensioners with an S1 (that last may have changed for those who weren't residents before the Withdrawal Agreement). We were still on the books with our GP until they received notification we had transferred our healthcare to Spain.
