Hi I work in the UK as a live in carer for 1wk and then home for 1 week. I am off ill. Does the doctor issue sick lines?

If you work and presumably live in the U.K. you would have to get your Medical Certificate from your NHS Doctor, really that has nothing to do with life in Spain.

Hi i don't live in UK. I work for 1 week as a live in carer which is my job in London. I'm from Scotland. Then I come home to Spain for 1 week which is where I am resident

Then it depends on where you are registered with a Doctor.
So if I understand you want a sick note from a Spanish doctor which will be accepted by your UK employer ?
Good luck with that one !
In any case in UK for absences of up to 7 days you don't need a sick note you self certify
