When filling out the health form do you attach your vaccination form ?
Or will the airport accept a print off ?

I had my EU certificate printed before I went to the UK and returned.
I came back on the 7th July and on the health form....if I remember rightly.....it asked if I had vaccines and then I remember a page that I clicked on EU digital certificate and underneath it said "other certificate".
I clicked EU certificate and it asked me to scan the QR code. I did but it didn't work for some reason then I clicked the underneath...other certificate and I put my date of birth, which vaccine I had etc. Then it gave me my QR code for the airport which was a relief, I was just about to go into a panick 🤣🤣🤣.
I'm not good with any forms 🤯

Hi
we came out 2 weeks ago from Wales, and printed the vaccination certificates, but also had to complete vaccination records on the locator forms, which when completed were on our mobiles as a QR code, but we still had to print out our vaccination certificates. It all went very smoothly thankfully. Hope this helps.
