As a Spanish resident you can use your  U.K. issued EHIC until expiry.

Once your card has expired  you can only obtain a new card  from the U.K. if you are a registered S1 holder, for the vast majority that means once you reach state pension age or a dependent of someone claiming S1 benefit.

Since Brexit if you  are a Spanish resident don’t fall under this category (eg you are under state pension age) you are no longer entitled to a U.K. issued card.

You would need to have paid into the Spanish system to qualify for a Spanish issued card.

So, in that case when your card expires,  if you were resident before the withdrawal agreement you can apply for an EHIC.

If you obtain residency after the withdrawal agreement you apply for a GHIC - which has the emergency cover in fewer countries.

Your current EHIC only covers you for the limited numbers of countries as well.

You need to show your S1 to obtain medical care in the U.K.

You obviously still need travel insurance .

Link below to apply for a new card which you can complete  up to 6 months before the old one expires.

Thank you.

Link to apply for a GHIC is above, it’s done on line and it’s one of the easiest  government forms you’ll ever complete. You’ll just need to start a new application for healthcare abroad, state you live in the EU, confirm you’ve got S1 and then your personal details. All done and dusted in less than 5 minutes.
