You get your card from the U.K. as long as you have S1 entitlement.
If you were resident pre Brexit you apply for the EHIC as it gives you cover in more countries. Since 01/01/2021 it’s the GHIC you need.

If you are visiting the UK then your original S1 forms are enough to get you free medical assistance.

Some people can apply for a new UK EHIC that they can use in Norway, Iceland, Liechtenstein and Switzerland, as well as in EU countries.

You cannot apply for GHIC if you are a UK National resident in the EU.

An S1 is only issued if you are a in receipt of UK State Pension and you are now resident in the EU. The S1 is to be registered with the relevant Social Security System in the EU country of residence. In Spain it is the INSS.

If you became a resident under the Withdrawal Agreement (you lived in Spain before the end of 2020) you do not have to pay for NHS treatment when visiting England. This includes S1 form holders living in the EU, Switzerland, Iceland, Norway or Lichtenstein, on or before 31 December 2020. Take a copy of your S1 form with you when you visit the UK. You may be asked to present this if you need to access treatment. If you no longer hold a copy, you can call Overseas Healthcare Services (0191 218 1999) and request a copy.

No need to register your S1 with a UK doctor. Just produce it along with your passport at the relevant time and place.
