Gold Car are notorious for this, as are many of the car hire companies operating out of Spanish airports. There is a global league for car hire services from airports and Spain always comes bottom. So much so that a few years ago the EU gave a warning to Spain to clean its act up in this respect, but nothing changed.
The thing is, most of the cowboy car hire firms charge peanuts for car hire, some about €50 a week for the base car. It's obvious they don't make any money at these prices, so they make it by either selling exorbitant insurance waivers or making false claims and charging unsuspecting customers. Having your own insurance cover doesn't mean anything to these guys, they don't care about the actual insurance, they just want to charge for "addons". So either you buy the addons or they just make a spurious charge against your card.
The best approach is to use a reputable company and always take the fully insured option. Yes, it will push the price to €150+ per week but it will mean don't get hit with extras.
