My car's ITV runs out middle of April.  I might not be here at the time so how long in advance can I have it done? Or how much time do I have after it runs out?  Any advice greatly appreciated.

One month before, not a day after. One month before will retain the same date for next year, eg due April 10th, get it done March 10th and next time will be due April 10th.

You cannot drive once the ITV has run out and should have it shipped by low loader to the ITV station. However, if you are a little over and have an appointment you are driving to it would have to be a cop whose missus burnt his toast that morning that would fine you.

And indeed it is just a fine, €200 or €100 if paid within 20 days. Cheaper than a guru whichever way you look at it!

For anything to do with driving in Spain have a look at the N332 website.   This site, and their Facebook page, is run in collaboration with officers of the Guardia Civil for English speaking people.   The site also has an English version of the Spanish equivalent of the Highway Code on it.
