Hi, how handy are you on the computer? I applied online to be registered on the health system using my S1 form earlier this year. It wasn't too difficult. I also did it for my partner and one of my neighbours. You need an electronic ID certificate on your computer or you can apply using a representative who has one and is willing to act on your behalf.

You need to send the downloaded application form, plus scanned copies of your passport, TIE card and S1 form. The social security in Denia usually reply by email within a couple of days with an A4 certificate saying you are eligible. You print that off and take it to reception at Pego health centre along with your other documents (must also take an up to date padron cert), and they issue you with your SIP card.

I'm not sure what you need translated, is it the application form?
