Have people had to give up health benefits to live in Spain

If you want to come and live here in Spain and apply for residency you have to have a years private healthcare insurance. So in effect  yes you would give up some UK benefits. However, there are some exportable benefits like carers allowance and certain disability benefits that could still be claimed, if eligible, even though you lived in Spain.

You don't get the attendance allowance, only the carers component is exportable. 

So you would lose any benefits on the AA.
