Free healthcare in Spain is only available if someone in your household is working and paying social security payments, either employed or self employed.
You also get free healthcare with the S1 form (EU wide form that transfers your healthcare to Spain and is paid by the country paying your pension). It is only available for those who have reached retirement age in their home country, for those with certain disabilities and certain other things like being pregnant.
You will have to show private medical cover to at least the standard given by the Spanish NHS until you are of retirement age and must cover pre-existing conditions.
After one year of residency, you can apply for the Convenio Especial which gives you full cover on the NHS but at a cost of 65 euro per person per month until you reach 65 when it goes up to 145 euro per person per month. This cost does not include the price of prescriptions.
Free healthcare for all residents was stopped back in about 2013 or so under the then PP government.
So, no, you won't get free healthcare by simply taking out residency. Note that if just one person in the household is working and paying social security then the whole household is covered.
