There are no brokers in Spain, as in the UK, who search the whole of the market; they are agents who typically sell the products of only one insurer. This means you have to do some legwork yourself to obtain multiple quotes for comparison. There are several factors that can affect the cost of cover, such as age and pre-existing conditions.
When we took out our health insurance 4 years ago, I obtained 6 quotes: we were aged 55 and 58, with no pre-existing conditions. We chose ASSSA - not the cheapest, but unlike some insurers, they don't apply age-related increases to your premiums so, despite having used the insurance, there has been no increase in our annual premiums, which total €1,780 p.a. for both of us.
It's worth noting that, for TIE/residency applications, you need to pay the first year's premium in full and have a certificate from your insurer to confirm that. Also, basic cover is not sufficient, neither is a policy that allows co-payments.
After the first year (and assuming Brexit won't affect our eligibility), you can switch to the Convenio Especial at a cost of €60 per person, per month (total for 2 people: €1,440 p.a.). This rises to €157 per person, per month at age 65, but when you reach UK State retirement age, you can apply for a Form S1 from NHS Business Services Authority in order to switch your UK health care rights to Spain, on the assumption that the S1 scheme is protected by the Brexit Withdrawal Agreement. Whilst prescription costs are extra, whether you have PHI or the CE, the advantage of the CE is that they don't exclude pre-existing conditions.
Some time ago, I detailed the insurance quotes I obtained 4 years ago - I'll see if I can find them and re-post it. Of course, the best advice you can receive is from those of us who have used the cover, as price is only one aspect. We're very happy with our cover from ASSSA.
