Assuming he's non-resident, your brother will need to take his EHIC & passport.

I'd like to register with a doctor so can anyone recommend an English speaking doctor in or around the Crevillente, Albatera, Catral area please. Thank you.

You first need to go to INSS to register your S1, and only then can you register with a doctor.
If you don't qualify for an S1, then you don't qualify for state healthcare.
