When people first arrive in Spain, it seems many purchase private health insurance.

When you later commence employment in Spain, and pay tax from your Spanish earnings.

Are you entitled to healthcare for you and your family immediately, or is there a waiting period before entitlement?

To gain a visa to be able to move to Spain permanently you have to have a full years private healthcare paid for the full year, hence those who newly arrived in Spain have private healthcare.

Any activity, employed or self-employed opens the door for becoming part of the National Social Security System which also includes health care. The "self employment" way may result cheaper and more efficient than a private healt insurance. Talk to a 2 asesor de empresa". As a side effect you can benefit from tax deductions (21% VAT on petrol among others).
