I am resident in Spain, but no longer working. I am under the retirement age and would like to register for healthcare under the Convenio Especial.

Can someone please tell me how to go about it? Do I need to apply at the Javea Centro Salud? Does anyone know the process?

To get the convenio especial you need to be a fully legal resident of at least one year and also have been on the Padron and had private healthcare insurance for that time. If you can't meet those conditions yet then that is the first stage. 

The Convenio Especial  the Spanish healthcare system that you contribute to every month and you get full healthcare even covering existing conditions. For under 65's I think it around €60 per month.

I'm not 100% sure but if you contact the local INSS healthcare department they will advise you.

Hi Kim,

The procedure for applying and the application form are available on the Valencian government website.  However, the English language option only translates the headings, so if you don't speak Spanish, either open it in Google Chrome, or use Google Translate:

You'll also need to complete a direct debit form for the monthly payments, which is available here:

Each person applying has to make an individual application, including the DD form. 

In addition to your completed application form and DD form, you'll also need copies of your residency certificate, passport, NIE and Padron certificate.  You'll also need a "legislation letter" from the UK - this extract from the UK government website explains:

"You’ll also need to show a letter stating that you are not covered by the UK for healthcare (documento de no exportación). You can request this by calling NHS Overseas Healthcare Services.

Monday to Friday, 8am to 6pm
Saturday, 9am to 3pm"

We have heard of some people being denied a legislation letter; in that case, ask them for an S1 and when they tell you that you don't qualify, tell them that's why you want a legislation letter!  ;o)

Once you've got all your documents together, I'd suggest contacting your Ayuntamiento (Town Hall) to see whether they progress these applications (in my area, I know that Rojales does this for residents).  It would also be worth asking them whether you need certified copies of your documents.  If your Ayuntamiento doesn't process these applications, you'll need to post it, or submit it in person (Covid-19 allowing) to:

One other thought - do you have a green residency certificate, or have you already swapped it for a TIE?  The green certificate is supposed to be valid, even after the end of the current Brexit transition period, so hopefully you won't be pushed back for not having a TIE (we've heard of this happening with driving licence exchanges).

Hi Kimmy,

Well. This was an eye opener. I went to the INSS in Denia and could not get in without an appointment because of Covid. The guy gave me a paper with a telephone number and a web address   http://www.seg-social.es 

The telephone number gave a message whenever you called that you cannot make an appointment by phone. Now, this is important. You have to go on the website at 9:00 in the morning precise! This way you will be able to get an appointment, but they are gone within minutes! So I managed to get a telephone appointment as all the others were gone. 

I do not speak fluent Spanish so I managed to get to speak to the same person in the office later that day as all was not very clear.

In my case I do not need the convenio  after all as I have a working history in Spain for a number of years and am therefore covered. For people that are not, they will need to go to the INSS.

Thanks for your feedback Kim.  If you don't mind me asking, how many years work record did you need to have in Spain, in order to qualify for State health care?  I assume this was work undertaken with a formal contract?

As you say, people wanting to join the Convenio Especial will have to go to the INSS, if their town hall doesn't process these applications, but it would be good to know what the qualifying period is for others who may be eligible for State health care via the work route.

I worked 4 years under a contract and was self-employed for 1 1/2 years. The clerk said it would be a different matter if you were 3 months in the country, but seem to imply that as long as you are in the system you will qualify until retirement age.

Thanks Kim, that makes sense - 4 years plus 18 months takes you over the 5 years' permanent residence for "settled status".  Good to hear of someone doing it properly and getting what you're entitled to 👍
