Start with your local GP ... choose one who speaks English.
Ensure that you select an insurance company that your local GP deals with, and which uses hospitals/clinics in your area.
It might also be important to you to choose an insurance company that includes interpretation services.
Your chosen insurance company will provide a plastic card to you (a bit like a credit card).
Visit your GP and register. He/she will record you. He/she will prescribe what you need (but if you have pre-existing medication, take your records from the UK to justify your need for the medication, as you will be unknown to the Spanish GP). Each time you see your GP, he/she will need your plastic card.
You will need to pay for your prescriptions (at a pharmacy).
An issue to consider .... pre-existing conditions may prevent you from obtaining private health insurance, or those conditions may be excluded.
