The form you have started to complete is correct, it’s the Spanish Travel Health Forn

It’s commonly known as the locator form for a couple of reasons, one because it asks for your location in Spain and secondly the U.K. form  ( no longer required) was called a  passenger locator form.

Most people found it easier to call the forms required for travel, locator forms. Up until recently every country required you to complete a form to gain entry and they were called a multitude of names

Your Spanish form can only be fully completed within 48 hours of your arrival date in Spain

Thank you...so when we apply for the QR code we input a Spanish address.

Yes you do need to fill out a passenger locator form to get into Spain. They also asked to see my vaccination status. That was 28th March

Rules for entry to Spain are different if you are coming from the EU, the original poster was asking about a journey from the UK.  Rules are different as well if you are a resident. 

However no matter which country you are travelling from, you need  to complete the SpTH form to enter Spain.  Applies to everyone flying into Spain or arriving at a Spanish port.
You were extremely lucky that it wasn’t checked at both airports in Belgium and Spain as you could have received a huge fine

Your best bet is to download the SpTH app. In there you can upload your vaccination details (which you can get from the UK NHS app) which are then stored for future trips. You can also start entering the details for your upcoming trip but cannot complete the form until 48 hours before travelling. Once you've completed it you will get a QR code which they scan on arrival in Spain. I'd recommend printing a copy as a backup in case your phone fails. Oh and you can put multiple people into the app so you don't have to have each person with an app on their phone.
