Convenio especial

I have been living in spain since march last year and going throu my residency, i just want to know where i need to get a form and address to send off for my convenio especial for the valencia region.

I have private medical ins but was told that after being here for over 1 year in my town i can apply for convenio especial as its cheaper than what i am currenty paying. Any advice would be appreciated. Thanks
