I have friends who travel in their camper van regularly to Schengen countries. All within the 90 day rules. Could anyone tell me if by getting some sort of visa they will be able to come for longer. Their main trips are to Spain and France. My friend has cancer and can’t get insurance. Hope someone can help!

Unfortunately there is not any long stay/extended stay visas.

If your friends want to stay past the 90 days they will need some sort of visa, probably the non lucrative visa. Proof of income and healthcare is required. 

If your friends can obtain an Irish passport they can then stay longer than 90 days by signing the foreigners register at the Local Police station, they would need to prove income and healthcare. 

Overstaying the 90 days could lead to serious consequences.
