Trying to translate a Spanish customs form

Can anyone help me with this? I have a Spanish customs form for a gift someone sent me from Australia and I can't receive the gift until I have filled out a customs form. I've managed to translate most of it but it's the bit at bottom I'm not sure of. I need to get this right as I won't receive the gift if I don't! I'm assuming it's the date but there's too many spaces for dd/mm/year.  Also the DNI del firmante o CIF de la empress. Any help would be appreciated!

The box starting EN you should put your location, date, month and year.

You are right with your signature, the other box DNI you need to put your NIE .
