Has anyone claimed income tax refund in Spain ? Is there a specific form to submit ?

To claim a refund you will have to fill a complementary return form /declaracion complementaria) with actual values. If accepted the money will be automatically credited to your account. The time limit is 4 years.

Thank you for this, hacienda website ( agencia tributaria) has a supplementary return form, not complementary. Is this the same form ?
