If a Canadian moves permanently to Spain would he/she have to take the Spanish driving test and medical in order to swap their Canadian driving licence to a Spanish one?

Yes because there is no direct agreement with Spain on exchanging licences. Each province in Canada has its own licence. You can drive with a Canadian licence and international driving permit for a maximum of 6 months in one year as a non resident (2 lots of 3 months) but you can't exchange the licence for a Spanish national one so you have to take a test.

You can drive in Spain with your valid Canadian driver's licence and an international driving permit for up to 6 months. For stays longer than 6 months, you must obtain a local driver's licence.
