I'd guess it's probably a form the company have cobbled together from the Valencian online form here:
None of the AEAT forms come anywhere near that description.
Tell your friend not to sign it until she's tried obtaining it herself. If she can't get an electronic certificate, or isn't computer literate, tell her to go to an AEAT office with her passport and a copy of her last tax return, and request it in person.

Thx Jim. I emailed a further query to her conveyancer, saying it's not clear from the form which certificate they're requesting on my friend's behalf: Certificado de Residente, Certificado de Residente Fiscal, or Agencia Tributaria Censal report? If it's the first, this can be obtained online for less than Eu 8, and the other two can be obtained directly from AEAT at no cost. As the conveyancer's company also act as my friend's tax advisors and presumably have electronic access to her tax account online, I'm at a loss to understand why any of these certificates would be charged at Eu 175. I await their response......
In the meantime, I've used the link in your guide and worked my way to "Request in person at the Tax Agency offices", in the hope of finding a list of office addresses - no luck, but Googling nearest AEAT office brought up Guardamar SUMA office, so if necessary, I'll take her there to get it and save Eu 175!

Careful Kim, you're mixing up Suma and Agencia Tributaria.
The nearest Agencia Tributaria is Orihuela, followed by Elche. I've always gone to Orihuela, even though it's in the old town and parking can be difficult.
Incidentally, for close friends I have on occasion got them to go to a town hall etc to get the code for an electronic certificate, got and installed the certificate, and then done things on their behalf.

Thx Jim, I was surprised that the SUMA office was offered - the Google search for AEAT offices also offered me Guardamar town hall, so I'll ignore them both! ;o) Elche is nearer for me and I know the city well, so I'll check that one out. Thanks again.
