Hi Everyone 

My daughter and I are visiting my house in Spain on 12/11. We're both very bad with technology and I'm told the locator form is tricky to complete. Can anyone advise please. Also what tests do I need to enter Spain?

There are no tests required at the moment if you are both fully vaccinated. You need to bring proof of vaccination, with you on your mobile or in hardcopy, probably best to have a hardcopy even if you have it on your mobile. 

The Spanish Health Form can be found at the link below along with a lot of information to help you complete.

  It is relatively straightforward and you can start filling it in now but the form can only be completed within 72 hours of travel. There is a helpline number if you have problems. When you complete the form they will send you a QR code which you need to show on arrival in Spain. It can be shown on your mobile or in a hardcopy, as with the vacc. details it is a good idea to have a hardcopy anyhow.

 The only problem I had with the form was that I could not find the area where I live on the list displayed, I just selected Orihuela (as we are in Orihuela Costa), it was excepted without any issue. 

  When you depart the UK the airline will want to see you have completed the Spanish Health form and that you have your vaccination certificate when you check in for your flight.

Going back to the UK you have to fill in the UK passenger locator form and book a day2 lateral flow test. The reference number you get when you book the test needs to be entered on the passenger locator form.

 As with the Spanish form you can start filling in the UK form in advance, but can only be completed within 48 hours on travel.

 Again, the airline with want to see your completed UK form and it is a good idea to have your vaccination certificate to hand though they should not need to see it.

         the Spanish health form can be found from the first link in my post above  where it says "new form/Individual FCS form".

Hi July, the form can be a tad tricky, just take your time, have your passport numbers ready, flight number and seat numbers.

You can start the form anytime, but it only allows you to go so far,  then you will be allowed complete the rest of it within 48 hours of your flight. 

You will also have to fill in a location form for returning to the UK, same sort of thing.
